The entry level professional massage therapist training program at WMTI was ended at the end of January, 2011 when the last of our students was graduated. Official transcripts were delivered to the Illinois State Board of Education (ISBE) as of February 28, 2011. To request a copy of your transcript contact the ISBE in writing with the following information:

• Name of school - Wellness & Massage Training Institute
• Probable dates of attendance
• Address to send transcripts to
• A valid signature is required.

Note: ISBE can send a sealed transcript if required. Specify recipient(s) in your letter.

There is no charge of the transcripts from the ISBE.
